Please send the Florida Tech Athletic Training Department a front and back copy of your current and functioning insurance card. Please take care to assure the card is readable and easily processed. Any copies that are not enlarged to 120% or come out with an appearance or background that is dark do not process well and another copy will be requested. PPO policies are preferred as the student athlete has a much easier and functional ability to see our team doctors w/o insurance issues prohibiting and/or delaying the visit, which in turn delays the student athlete’s return to play. If you have an HMO, please read and follow the below instructions and include this information when you send the front and back copy of the card.

Please be aware that if you have an HMO:

The Athletic Training Staff will need a letter from your insurance company that states coverage is effective in all geographical areas in Florida for medical & athletic injuries/ illnesses while your child is a student athlete here at Florida Tech. If no proper notification is received, then the athlete and parents / guardians will be held financially responsible for any medical bills that may occur.

There also cannot be any clause that excludes diagnosis related to a sports injury. As the parent, your health plan will be the primary insurance. The intercollegiate sports insurance (Markel Pioneer) is the secondary health plan. The intercollegiate sports plan (has a $1000 deductible) will not begin to pick up any charges until after the first $1000 worth of medical expenses has been paid or after your primary insurances deductible has been met.

The doctors we use are MIMA Physicians who are contracted with the following health plans:
Accountable Health Plans, (Also known as – IHG- Inter Plan Health Group PPO)
AETNA – PPO, POS, EPO, HMO- (HMO plans will require auth from PCP)
BCBS – PPO, PPS, HMO, HEALTH OPTIONS, BLUE OPTIONS

TRICARE STANDARD (we do not participate with Tricare Prime)

Please look at the front and back of insurance cards. Most insurance cards that require a referral/authorization to see a specialist, will state it on the member’s card.
